Temporomandibular Disorders Are Not a Single Condition
“TMJ” is used loosely to describe any jaw complaint, but temporomandibular disorders are a group of distinct conditions with different treatments. Distinguishing between them determines whether an orthotic, physical medicine, medication, or joint-directed treatment is appropriate:
Masticatory Muscle Disorders
Pain arising from the muscles of chewing rather than the joint itself. Often associated with parafunctional clenching, and frequently the source of pain mistaken for dental or sinus origin.
Disc Displacement
The articular disc moves out of normal position, producing clicking, catching, or episodes of limited opening. Displacement with and without reduction behave differently and are managed differently.
Degenerative Joint Disease
Arthritic change within the temporomandibular joint, which may produce crepitus, loading pain, and altered function, and which requires a distinct management approach.
Diagnosis follows validated criteria and, where indicated, imaging. Management is conservative and reversible before anything irreversible is considered — see TMD treatment for the treatment pathway.
